What It Is
Hexarelin is the most potent GHS-R1a agonist of the first-generation GHRPs, producing the highest GH release per dose. It also binds the CD36 scavenger receptor (now called the hexarelin receptor) independently of GH release, producing direct cardioprotective effects including reduced cardiac fibrosis, improved ejection fraction, and protection from ischemic injury. Like GHRP-2, it raises cortisol and ACTH. Hexarelin desensitizes more rapidly than other GHRPs with continuous use, necessitating cycling. It activates PI3K/Akt survival signaling in cardiomyocytes directly via CD36.

Dosing Protocols
Timing: Before bed or post-workout. Strict cycling required (4-6 weeks on, 4 weeks off) due to rapid desensitization; more potent than GHRP-2 per mcg
↓ Apply these values to the reconstitution calculatorTiming: Morning fasted and before bed. Combine with CJC-1295 for maximum effect; must cycle off to prevent desensitization
↓ Apply these values to the reconstitution calculatorTiming: Morning, post-workout, bedtime. Maximum potency GH secretagogue; desensitization still occurs; cardioprotective benefits occur even at low doses via CD36

| Week | Expected Effects |
|---|---|
| Week 1 | Very strong GH pulses; significant water retention; cortisol elevation |
| Week 2 | Early GH-driven body composition changes; improved recovery; early desensitization warning |
| Week 4 | Peak GH stimulation effect before desensitization becomes significant; strong body composition improvement |
| Week 6 | Cycle off to allow receptor recovery; cardioprotective benefits from CD36 continue with lower doses |

Key Research Papers
Demonstrated Hexarelin's direct cardioprotective effects through CD36 receptor, independent of GH release, including reduced myocardial fibrosis and improved cardiac function.

The most potent GHRP paired with GHRH analog produces maximum GH pulses. For short-cycle maximum GH output this is the strongest combination.
Hexarelin's cardioprotective effects combined with BPC-157's vascular and tissue repair effects provide comprehensive cardiovascular protection.
Frequently Asked Questions
Yes - Hexarelin is the most potent GHS-R1a agonist of the first-generation GHRPs, producing the highest GH release per microgram. However, it also desensitizes faster than others, requiring strict cycling.
Hexarelin has a unique direct cardioprotective mechanism via CD36 receptor binding that is independent of GH release. This makes it useful for cardiac protection and recovery even in cases where GH stimulation is not the primary goal.
